Zilele libere se acord\u0103 la cererea unuia dintre p\u0103rin\u021bi, depus\u0103 la angajatorul persoanei care va supraveghea copilul \u00een perioada suspend\u0103rii cursurilor sau \u00eenchiderii temporare a unit\u0103\u021bilor de \u00eenv\u0103\u021b\u0103m\u00e2nt.<\/p>\n<p>2. Cererea va fi \u00eenso\u021bit\u0103 de:<\/p>\n<ul class=\"ul-char7\">\n<li style=\"list-style-type: none\">\n<ul class=\"ul-char7\">\n<li>declara\u021bie pe propria r\u0103spundere a celuilalt p\u0103rinte, din care s\u0103 rezulte c\u0103 acesta nu a solicitat la locul s\u0103u de munc\u0103 zile libere ce i s-ar cuveni potrivit Legii nr. 19\/2020, respectiv c\u0103 nu se afl\u0103 \u00een concediu de odihn\u0103 sau f\u0103r\u0103 plat\u0103;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<ul class=\"ul-char7\">\n<li>copie a certificatului\/certificatelor de na\u0219tere al\/ale copilului\/copiilor.<\/li>\n<\/ul>\n<p>3. Modelul declara\u021biei este prev\u0103zut \u00een anexa la HG nr. 217\/2020.<\/p>\n<p>4. Pentru decontarea sumelor pentru plata indemniza\u021biei, angajatorul depune cerere la AJOFM\/mun. Bucure\u0219ti, \u00eenso\u021bit\u0103 de urm\u0103toarele documente:<\/p>\n<ol class=\"litera-mica-paranteza\">\n<li style=\"list-style-type: none\">\n<ol class=\"litera-mica-paranteza\">\n<li>lista angaja\u021bilor care au beneficiat de zile libere \u0219i indemniza\u021bia acordat\u0103 pe aceast\u0103 perioad\u0103;<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<ol class=\"litera-mica-paranteza\">\n<li style=\"list-style-type: none\">\n<ol class=\"litera-mica-paranteza\">\n<li>copii de pe statele de plat\u0103 din care s\u0103 reias\u0103 acordarea indemniza\u021biei;<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<ol class=\"litera-mica-paranteza\">\n<li style=\"list-style-type: none\">\n<ol class=\"litera-mica-paranteza\">\n<li>declara\u021bie pe propria r\u0103spundere a reprezentantului legal al angajatorului prin care se atest\u0103 c\u0103 lista de la lit. a) con\u021bine persoanele care \u00eendeplinesc condi\u021biile legale pentru a beneficia de indemniza\u021bie;<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<ol class=\"litera-mica-paranteza\">\n<li>dovada pl\u0103\u021bii contribu\u021biilor \u0219i impozitelor aferente lunii \u00een care s-a pl\u0103tit indemniza\u021bia.<\/li>\n<\/ol>\n<p>5. Documentele se transmit prin po\u0219t\u0103 electronic\u0103 (sau prin alt mijloc de comunicare \u00een situa\u021bia \u00een care nu pot fi transmise prin po\u0219ta electronic\u0103) \u00ee<strong>n cel mult 30 de zile de la data efectu\u0103rii pl\u0103\u021bii contribu\u021biilor \u0219i impozitelor aferente indemniza\u021biei<\/strong>.<\/p>\n<p>6.
